docs(skill-writer): sharpen user-invocation note and worked example
- Clarify that disable-model-invocation is honoured per-agent (the skill fires by name where supported), rather than implying it universally blocks reach. - Rewrite the worked-example explanation to name each lever applied (no-op cut, completion criterion, information-hierarchy demotion behind a context pointer).
