Add vaccination refusal to Viewer & db

What needs to be done

Philly is interested in seeing vaccination refusal data in both the Viewer and the database.

Why it needs to be done

UAT request

Acceptance Criteria

• Research how vaccination refusal appears in eCRs (CDA format) & confirm with Peri
• If necessary, add vaccination refusal data to the Viewer (may or may not be converting this information already)
• If necessary, add vaccination refusal data to ecr_immunizations table in the database. Would like to see, yes/no if refused, and if refused, reason for refusal (if exists).
• Add/update any relevant tests
