After so many years some repeat visitors get haggard because they don't actively fulfill their needs unless they're visiting a particular location (e.g. temples for praying, library for abstract thinking, guild halls for learning). But visitors to a tavern will never drink (unless served) or eat.
Obviously they'll still have a lot of unfulfilled needs, but at least this will mitigate some of the effects. And players usually have so much surplus food and drinks it shouldn't really be a problem if visitors grab some for themselves (they already "steal" library books and tavern instruments, after all).
