chore(beads): file 356d5 + 4b6un — wave-3 follow-up consolidations

Two more rollup beads from the cass audit pass 2:

  - 356d5 — Wave-3 follow-up consolidating three TUI test gaps
    (2dccg.11.1, 2dccg.11.2, 2dccg.4.18.12) into a single
    actionable bead so they can be addressed together rather
    than re-derived per-bead.
  - 4b6un — Wave-3 follow-up consolidating five
    telemetry / E2E gaps (c4of, z9fse.10, z9fse.3.6, 3a51,
    q14z). Same rationale: similar scope, similar fixtures.

Both beads link back to their predecessors via the
`completion-debt` prefix in the title and carry the audit-pass-2
evidence pointer in their description.
