duplicate polls listed on user profile and user activities page

on the user profile page i have found that the same poll is listed twice, the user has created one poll and participated in one poll, but they are the same poll..

is it possible that we are counting these activities twice and showing two entries? we should only show unique polls in both the user profile view and user activities view...

make a new poll
vote on the same poll
ensure poll does not appear twice on user profile or user activites page

if this is approved we can move it to done
