feature request: ability to specify starting index for destination addresses

hi,

firstly: I love this pluging. Thanks so much for making it. It really helps preserving privacy.

I have a problem: my target wallet has some frozen addresses. They were never used and I don't want to use them as target for an inter wallet transfer I want to do. Background: they're registered cashaccount addresses.

Now I can see that it might break the design to open the destination wallet file to determine "frozen" status of addresses, so maybe as a workaround a starting address could be given when generating addresses in the destination wallet?
