`Residing` place may not be the same as `Done` place

When traveling back from a trip home the Residing address is not the same as Done address.

I fulfilled this form https://media.interieur.gouv.fr/deplacement-vers-france-covid-19/index-en.html and put Minsk, Belarus as my Residing address and the generated PDF now has Minsk in a Done place field, however, I'm populating the form in the US.
