Subject: [PATCH] Update transfer-phone-numbers-to-teams.md

Modify Note: "The CSV file must have only one column with a header named PhoneNumber. Each phone number must be on a separate row and can be digits only or in E.164 format." to "List the phone numbers you want to port in a CSV file that has one column with no header and one phone number per row in E.164 format." as per info in TAC
