Gateway header value passing is incompatible with Cloudflare

Cloudflare does not support dots . in header keys which is used on NServiceBus headers. To make it compatible with Cloudflare either the header passing must be changed or should be customizable.

Spent some time looking at this and it looks like this is a known (and accepted) issue in nginx, which I think is what Cloudflare is using for their reverse proxy. More info can be found in the nginx ticket system here: https://trac.nginx.org/nginx/ticket/629

Based on the response in that issue, nginx is never going to support . in header keys. Since nginx has become the defacto standard for reverse proxy, we should look into a mechanism that will work for that platform.
