make handNotes member of header

The TEI Lite group has determined that the <handNotes> element is a member of the 'transcr' module. The only place where <handNotes> s allowed is within <profileDesc> and its child (<handNote>) is also a member of the "header" module. Therefore we propose to make <handNotes> a member of "header".

If <handNotes> were a member of "header", then it would be available without the inclusion of the PH chapter, which has all the other stuff for handwritten documents. I am inclined to say it should remain where it is.
