Implement Configurable Instance Number in Simulations for gPBFT Package

Simulations always start at instance zero. Most tests also implicitly assume this. But the gPBFT package allows a user to start at a non-zero instance.

Implement the ability to configure instance number in simulations and add additional tests to exercise relevant code.

For completeness we should have at least one test that touches this path. But it is certainly low priority. I have removed it from the milestone and labeled it appropriately.

@masih : I have added this to MX: Priority and sequencing TBD since I'm assuming this isn't needed anytime between now and shortly after f3 activation. Let me know if that is wrong.
