make sure transition between indexing and streaming is smooth

We need to add code that makes sure we smoothly transition between past blocks that were downloaded from bitcoind and new blocks that are streamed using zeromq, so we can guarantee we always receive the next block and/or re-orgs are handled correctly in this transition if they do happen.
