tasksCreate() callback not running.

I notice that every 20th or so worker.tasksCreate() call fails to actually create the given task.. And the callback never gets run with an error explaining anything.

        worker.tasksCreate('exercise_starter', {}, {
          delay: 300
        }, function(e, data) { 
          console.log('this is not run sometimes..')
          console.log(e, data) 
        })
