docs: STATUS — record worktree sync + correct next-session cwd

- Update commit reference d416f78 → de33bcb (Phase 2 merge that
  brought Tier 4 + v1.2 plan into main).
- Note that v1.1 worktree at C:/Projeler/mnemos-v1.1 was
  fast-forwarded to de33bcb so junctions + editable install see the
  English skills.
- Explicitly tell the next session to open in C:/Projeler/mnemos-v1.1
  (not C:/Projeler/mnemos) — junctions point there.
- Drop the stale "Tier 4 in-flight" pending action; Tier 4 is merged.
