Remove ethSubInterface

I'm not sure that this works the way that it was hoped that it should, and so I think that we should remove it at this time and just use the base ianaift identities.

If you agree, then I think that this should be a trivial change to remove (but also need to check whether it applies to the sub-interface draft as well).
