Worker w3 on memracer task 64 (better-auth Phase 2). coder_propose_plan submitted 4x; mediator jobs 221, 225, 227 each returned 'job handler timed out' (job 221 also, plus two 'pending' polls that then failed). Plan text ~2-4KB. No verdict returned, so implementation is blocked at the approval gate.
Repro: submit a moderately large plan (multi-paragraph, ~15 files_touched) for a P2 task; mediator review job times out rather than returning approved/revise.
Impact: cannot start work without an approved plan. Worker will release the task to requeue and pull other work.
Suspect: mediator model latency/timeout budget too tight for large plan payloads, or the review job wedged.
