Formatted CONTRIBUTING.md.

- Passes `markdownlint` (mostly)
- Removed `$` from shell instructions, as to be more copy/paste friendly
- Annotated code-like things with backticks.
- Annotated all shell blocks with `sh` to format nicely on GitHub.com.
