Issue: Add medication adherence tracking (Taken / Missed)

Currently, medication reminders notify users, but the system does not track
whether a medicine was actually taken or missed.

Proposed enhancement:

• Add "Taken" / "Missed" action on reminder notification or dashboard
• Store adherence status in database with timestamp
• Enable basic adherence history per medication

This will improve medical usefulness and enable future analytics.
