Improve documentation on NewCreateIndex

Hi, I'm creating indexes with the ORM, and it's all good, but it took a lot of searching and some trial and error. The documentation seems very poor about this, I believe a few complex examples with the NewCreateIndex function would help a lot, especially with the counterpart SQL code.

Additionally I found that these things are pieces of go-pg code, so the documentation is there... kinda.

Can any maintainer tune this bot? The issue is relevant, like the others, and I believe it may help people to better use this library. If the issue tracker is kept only for aesthetical purposes, at least make it clear so nobody else wastes time keeping issues open.
