Gen Info - New Feature (ZIP Lookup)

Guide the user to be able to select the appropriate ZIP code by either or both:
Provide a drop down ZIP code list filtered based on the City, State inputted on the Gen Info page.
Provide a pop-up window that allows the user to look at a list of zip code by inputting state and city.
