Purpose of this pull request

This pull request (PR):

  • Restores the contents of the App Development index page
  • Establishes the index file of the Start Kit directory as the one will be managed in Helix
  • Moves the contents of the former starter kit index file into a new create-integration.md files
  • Removes the dummy file I had set up for the left nav. Since I'm using headings without expanding sections, I had to duplicate some entries. I checked that next topic/previous topic links work as expected.
