Add web UI for reporting, driver responses, and admin controls

Summary

• build a new index page with reporting, public map, driver portal, and admin role management sections
• implement frontend logic for anonymized map markers, driver verification/responses, and moderation queue controls
• refresh shared styling for responsive forms, panels, and feeds that handle evidence attachments and notifications
