Implement the fix suggested by @pricerc in #178 to use
OUTPUT ... INTOfor INSERTing in MSSQL providerNot sure if I have handled the tests for other DB providers correctly (I have tried to create a table with triggers where possible, but I'm not very experienced with any DB's other than MSSQL). If preferred I can add the test directly to
PetaPoco.Tests.Integration/Databases/SqlServerTests/SqlServerInsertTests.cs
