Fixes #269.

Summary:

Ignore wiki-link matches whose opening or closing marker is inside an inline/fenced code span when references_in_codeblocks is disabled.
Preserve normal wiki-link parsing outside code spans.
Add regression tests for literal [[ / ]] markers in inline code.

Verification:

cargo fmt --check
cargo test vault_tests
cargo test
