Unify Relative Links

• Removed unnecessary .md extension which also fixes users
  ending up on .html pages instead of just the name, as is intended.
  ( /Page.md -> /Page.html , /Page -> /Page ( .html ) )
• Unified all relative links to use ./ syntax instead of standalone
  to make them all the same and easier to differentiate which are which.
