Repair broken setup anchors and standalone remediation references, then validate local links, heading anchors, skill isolation and source/Kiro parity in CI. Keep the reference fixes, parser dependencies and regression coverage together.
Split from #87. This includes the setup-anchor correction shown in that review; remediation-framework changes remain in #87.
Validation: 95 deterministic tests passed; reference checks report no findings, removing all 10 baseline findings; Markdown checks passed.
