Count only consecutive LLM API errors against CodeAct max_retries - #507
CodeAct and PurePython end a call after max_retries LLM API errors, but they count them for the whole call and never resets the count after a successful LLM call. In a long turn, a few unrelated transient endpoint errors add up and kill it, even though every retry succeeded.
How it happened (Garph, session e5945025, Kimi K3 via inference-api.nvidia.com): the gateway returned a transient 400 ('max_tokens' is too large: 262144, a gateway fallback bug; NOOA sent 32768) at iterations 0, 6 and 7. The retry after each one succeeded and the agent kept working. The third error still hit max_retries=3 and the turn ended with LLM API error after 3 retries.
Fix: in both CodeAct and PurePython, LLM API errors get their own counter, consecutive_api_errors, which resets after every successful LLM call. A call fails once max_retries API errors happen in a row, as the CodeAct max_retries docstring already described ("Maximum consecutive errors before failure").
API errors no longer add to error_count. The other errors that do (invalid tool arguments, unknown tools, empty code, empty replies, failed result validation, PurePython XML format errors) still accumulate as before. This PR only changes how endpoint failures are counted.
PurePython counts both of its API-error paths this way: httpx timeouts and other exceptions. An XMLFormatError resets the API-error counter, because the model did reply.
The error events and log lines report the consecutive count, and the per-turn debug log shows api_err= beside err=.
The CodeAct skill doc (skills/nooa-codeact-advanced/SKILL.md) and CHANGELOG.md describe the new behavior.

Tests added/updated and passing: test_successful_call_resets_api_error_budget (one for CodeAct, one for PurePython) fails on main with LLM API error after 2 retries and passes here. test_llm_error_exhausts_max_retries still covers consecutive failures. Full unit suite (tests, excluding tests/integration): 9376 passed, 5 skipped, 1 failed. The failure, test_codeexec_method_parenting.py::test_method_called_from_code_nests_under_code_execution, is flaky and fails intermittently on main without this change.

CodeAct and PurePython now count consecutive LLM API errors toward the retry limit. A successful model response resets that count, so intermittent API errors do not exhaust the limit.
Other errors, including invalid tool calls and failed result validation, continue to accumulate toward the retry limit.
Documentation
Clarified how the retry limit applies to consecutive API errors and other errors.
CodeAct and PurePython now track consecutive LLM API errors separately from other errors. Successful model responses reset the API-error count. Both strategies use this count for API-error retry limits. New tests cover recovery after separated API failures.

Both session types add a consecutive API-error counter with increment, reset, and exhaustion checks. Documentation describes separate API-error and other-error counting rules.
CodeAct increments the API-error count for LLM API exceptions and resets it after a successful response. Prompt block syntax errors do not change the count. A test covers three separated API failures with max_retries=2.
PurePython counts timeouts and other LLM API exceptions against the consecutive API-error limit. XML format errors and successful code-generation responses reset the count. Tests cover separated API failures and an XML-format response between failures.
Some CodeAct turns may terminate under the wrong retry policy or retry excessively when non-API failures occur; correct the classification before merging.

Thanks, the fix looks right. I confirmed that the new test fails on main and passes on this branch. Could you also make these changes in this PR?
Update the CodeAct skill doc. skills/nooa-codeact-advanced/SKILL.md:44 still describes max_retries as a "Cumulative session error budget, not consecutive (the counter is never reset)" that includes LLM API errors. After this change, that's wrong, and agents read this skill. Please say that LLM API errors count only while they're consecutive and a successful LLM call resets them. Other errors still add up across the call.
Apply the same fix to PurePythonStrategy. src/nooa/strategies/pure_python.py (around lines 348–374 on main) charges LLM API errors to the shared error_count in the same way. Its long runs can fail from scattered transient errors just like CodeAct's. A matching consecutive counter with a reset after each successful call, plus a test like test_successful_call_resets_api_error_budget, would keep the two strategies consistent.
Add a CHANGELOG entry. This changes when CodeAct calls fail, so it belongs under ## [Unreleased] in CHANGELOG.md. For example:
CodeAct and PurePython max_retries now count only consecutive LLM API
errors; a successful LLM call resets the count. Other errors, such as
invalid tool calls and failed result validation, still add up across the call.
Two small gaps in codeact.py:
The per-turn debug log at the top of the loop (Loop iteration: iter=…, err=…) shows only error_count. Please add api_err={session.consecutive_api_errors}/{session.max_retries} so the new counter appears in logs.
BlockSyntaxError means the model did respond, but the except BlockSyntaxError branch doesn't reset the API-error counter. So "API error, syntax error, API error" still counts as two in a row. Calling session.reset_api_errors() in that branch before continue would fix it.
Address review on #507: PurePython gets the same consecutive LLM API
error counter as CodeAct (timeouts and other API errors; an XML format
error resets it because the model replied). Both strategies log the
counter in the per-turn debug line. Update the CodeAct skill doc and
the changelog.

@drazvan Thanks. Done in the latest push, except for one point:
Skill doc:skills/nooa-codeact-advanced/SKILL.md now describes both counters. LLM API errors count only while they're consecutive, and any model response resets them. Other errors add up across the call.
PurePython: same change. It has a consecutive_api_errors counter that resets after every successful LLM call. It covers both API-error paths there: httpx timeouts and the generic except Exception. An XMLFormatError also resets it, because that error comes from parsing a reply the model actually sent. A new test, TestPurePythonLLMApiError::test_successful_call_resets_api_error_budget, fails on main with LLM API error after 2 retries and passes here.
CHANGELOG: added under ## [Unreleased], using your wording.
Debug log: both strategies now log api_err=<n>/<max_retries> next to err= in the per-turn line. Both also gained an api_errors_exhausted() helper.
BlockSyntaxError: I didn't add the reset there. This error only comes from compile() when a Context(expr=...) is built. It's raised while the prompt is being built, before any LLM request goes out, so nothing in that attempt shows the endpoint is healthy. Take "API error, syntax error, API error": no model reply happens between the two API errors, so counting them as consecutive is correct. I added a comment in that branch saying so.
Add a regression test for API failure → XMLFormatError → API failure.
The existing tests cover API failures separated by valid responses and repeated XMLFormatError responses. They do not verify that an XMLFormatError resets the API-error counter before a later API failure. Add a focused strategy-level test for this sequence.

@coderabbitai Added in 2932cba as test_xml_format_error_resets_api_error_budget. Your suggested version used _resp(""), but an empty reply goes down the empty-code path and doesn't raise XMLFormatError. The test sends a nested-tag reply instead (<tool_code><code>...</code></tool_code>), which does raise it. The order is API error, XML format error, API error, success, with max_retries=2. The test fails with LLM API error after 2 retries when the reset is removed, and passes with it.
Do not charge non-API failures to the consecutive API-error budget.
The handler catches all exceptions from RuntimeActor.generate(), except the explicitly handled BlockSyntaxError. That operation includes prompt construction, middleware, request preparation, response parsing, context recovery, response enrichment, and event persistence.
The handler calls record_api_error() for each exception and raises after api_errors_exhausted(). It does not increment error_count. A successful response then resets the API counter before the next loop iteration. Therefore, intermittent non-API failures can avoid the cumulative max_retries limit, while consecutive non-API failures can terminate the turn as an LLM API failure.
Classify failures before updating counters. Use record_api_error() and api_errors_exhausted() only for genuine LLM API failures. Use record_error() for all other failures.

@furgalep Thanks for correcting the fixture. My _resp("") suggestion would not exercise XMLFormatError.
The nested-tag reply targets the intended error path. Your reported fail-without-reset and pass-with-reset results demonstrate that the test detects a missing API-error counter reset.
