Add GitHub Actions formatter

Summary

• add a --github output formatter for GitHub Actions annotations
• map Vint severities to error, warning, and notice workflow commands and escape workflow command payloads
• document the new CLI option and cover formatter/config selection paths

Fixes #396.

• git diff --check
• inspected the formatter output against GitHub Actions workflow command syntax
• Not run locally
