How to create Mautic Form

Mauticommerce is posting following data. So you should make a form that using match Mauticommerce's param in Mautic.

##1:Set up Mautic form

Create new form in Mautic.

##2:Create Fields Select "Fields" tab. You can make many type form field.

I think every form can be "Text" type.

##3:Select field label Input form field label.

Please set following params.

Mauticommerce POST Params
address1
address2
city
company
country
email
firstname
lastname
phone
zipcode
state
order_id

##4:Mapping Mautic Lead Field Select "Lead Field" Tab. And select lead Field.

##5:Make fields that you need Do Step2 -> Step4 while all fields that you need is created.

##5:Save Click "Save & Close" button.

You should note created Mautic form ID.

##6:Import Mautic Form ID in Mauticommerce Plugin Set following params in Mauticommerce setting page.

• Mautic URL
• Mautic Form ID( You noted in Step 5 )
