The mditer had a copy-pasta mishap.
The concepts were magical, this is now only the case for cdata_t.
Using element-wise ranges has been simplified by adding a constant object by_elem that you can pipe into.
Removed naming dynamic_extent for spans. This mostly helps symbols in debugging.
Changed ranged_grid_t to inherit from grid_t. Also made its template parameters simpler so that the symbols are shorter in debugging.
Made grid_t a full matpack-type.
Removed elemwise and flat_pos files as they are not used and their solutions are fixed by enumerate and zip
