CPS-0033 | Fix formatting of references

@maureenblack @daniellestanko-gov we didn't notice (I think since final review was focused on satisfying review points from the PR dialogue) that recently added footnotes were "stuck onto" one of the bulleted reference lists.

• As a result the footnotes still display correctly (since they can be defined anywhere in the document) but they leave an artefact of an empty bullet in that list.
• The resulting markdown (after this fix) will also have the source & markdown in the same ordering, which makes the document more maintainable.
