      • An internal-facing web application developed for VA healthcare staff to efficiently monitor and manage Veteran patients throughout every phase of the organ transplant process. Designed to enhance care coordination and ensure timely interventions, the application provides comprehensive tracking from initial referral through transplant evaluation, waitlisting, transplantation, post-operative care, and, if applicable, end-of-life follow-up.
