Remove various classes, part 1 #8371

This is part of #8364 .

This cleans out various useless classes.

be7397a : removes <div class="index">
8c72981 : removes the "note" class on <p> elements
bd975b5 : removes the "directory-tree" class on <ul> elements (this used to do something long ago)
40292c0 : removes div class="example">
2b1f4e4 : replaces all the code blocks in https://developer.mozilla.org/en-US/docs/Web/API/Presentation_API with non-mangled versions
f9fc369 : removes <span class="comment"> elements - it appears people think these are hidden?
