Added support for user provided image icons.

Hello,

Found your project and preferred it to other similar projects due to its light-weight nature.
However (like others) i found the icon selection insufficient for my needs, so i did a few changes.

I did the following:

• Added option to use a user provided raster image(e.g. png,jpg) as an icon instead of the previous font icons
• Made the new application entries a bit more consistent to the php generated entries(but still need some TLC to be identical).
• Splash screen and top navigation menu can use font(old) and image(new) icons.

I am not used to js/jquerry programming so if you want me to change anything let me know.
