This issue was due to
onDidChangeevents being rejected inExtHostDocumentContentProvider, see:https://github.com/microsoft/vscode/blob/5f65e5f6e99c4dd9e3bee943175663b728f0d8ea/src/vs/workbench/api/common/extHostDocumentContentProviders.ts#L55
We know for sure that it will be up to date when
onDidOpenTextDocumentis received, so trigger the event again in case any were dropped.
