Bundling es module dependencies

Someday in the future, es modules will be smoothly supported everywhere. Today isn't that day.

This release bundles dependencies so that the ecosystem can work.

Null geometries

The main element of this release is that Features with null geometries, which are allowed by the GeoJSON specification, are now properly supported in tokml. They will produce Placemarks with no geometry, which is supported by the KML specification.
