fix: harden named session routing

Summary

• bump ContextRelay package/plugin metadata to 1.1.2 and document the patch release
• preserve runtime session identity through Codex CLI env, Codex MCP live relay paths, and bridge disabled-state recovery
• route Codex explicit ContextRelay command responses/finality through the originating runtime session
• avoid global daemon idle shutdown while any runtime session still has connected clients
• scrub inherited ContextRelay runtime env from reconnect e2e daemon launches
