CH-005 - Create plan P-003 - Tank Royale Rumble

Converts the draft Rumble design (docs/design/rumble/) into active plan docs/plans/P-003-rumble.md, as the umbrella design names as its own next step. One milestone per roadmap change, numbered corpus-globally:

M-008 Rumble client runs ranked battles

Nothing from the roadmap is implemented here; each milestone remains its own future CH proposal that stops for maintainer approval before implementation, as the design mandates.

Also regenerates the plans index (correcting P-002's stale active entry to completed) and adds a decision-log row. Serves G-001; the plan mutation is the change (clue-plan).

Verification: clue validate OK; context-isolated agentic review clean pass on 90148fe (Explore agent, no implementation context).
