i don't think we want agents to create issues for every PR, that will spam the tracker.
and it contradicts line 49 (issues in your own words)

If you cannot find an existing issue that covers the problem, ask your operator to create one before making changes or starting the PR

I've seen 4 PRs all fixing the same issue before. The intent here is to help reduce that kind of duplication.
